OUR MISSION:

To identify strengths, gaps, and barriers that students with disabilities encounter while transitioning from the secondary educational environment to adult life.  The Transition Task Force provides suggestions and recommends solutions to increase opportunities for students with disabilities to reach their desired post-school outcomes through inter-agency collaboration.

MEMBERSHIP:
Agency for Persons with Disabilities (APD)  The Agency for Persons with Disabilities assists people who have developmental disabilities and their families.  APD also provides assistance to identify the needs of people with developmental disabilities and funding to purchase supports and services. Florida Family Care Council  Advocates, educates, and empowers individuals with developmental disabilities and their families, partnering with the Agency for Persons with Disabilities, to bring quality services to individuals for dignity and choice.
Alliance for Human Services a non-profit organization dedicated to the development of a Health and Social Services Master Plan (SSMP) for Miami-Dade County. Florida International University (FIU)  Disability Resource Center(DRC)  The DRC is the department within the University responsible for providing students with disabilities, with the necessary support to successfully complete their education and participate in activities available to all students.
Business Leadership Network (BLN) The Miami-Dade Business Leadership Network (BLN) is an employer-to-employer network with a common vision to increase workforce diversity through the recruitment and hiring of qualified workers with disabilities. Jewish Community Services of South Florida, Inc. (JCS) An agency that provides exemplary social services in South Florida on behalf of the Jewish community which promotes the quality of life and the maximum achievement of potential within that community and the community as a whole.
Center for Independent Living of South Florida Assists anyone with a disability to achieve and maintain an independent and self-determined lifestyle to the maximum extent possible and to vigorously advocate for systems change that results in equality, dignity, and freedom of choice, and eliminates barriers that prevent people with disabilities from achieving their full potential. Miami Dade College (MDC)  Miami Dade College provides accessible, affordable, high quality education by keeping the learner's needs at the center of decision-making and working in partnership with its dynamic, multi-cultural community.  MDC offers a full complement of programs, services, and activities to meet the educational, cultural, and training needs of our community.
Commission on Disability Issues (CODI) The primary role of CODI is to advise the County Commission on County government issues that affect people with disabilities. Miami-Dade County Public Schools  (MDCPS)Provides and supports the highest quality education for our diverse community of children and adult learners
bullet Adult/Vocational and Alternative Education, Division of Special Education, Special Programs-Juvenile Justice Support Office, Regional Centers, Student Services, Non-Instructional Staffing, Compensation Administration, and Lindsey Hopkins Tech-Dual Enrollment Program
Community Committee for Developmental Handicaps (CCDH) Provides services to people with physical, developmental, and sensory disabilities.  A coalition of consumers; families; advocates; community service providers; and other public, private and governmental organizations. Miami Dade Transit (MDT)  The largest transit agency in the state of Florida! We strive daily to keep our Metrobus, Metrorail, Metromover, and Paratransit services safe, reliable, and courteous. We are especially proud to be an accessible system offering mobility to people with disabilities. After all, every customer is our number-one priority. Division of Vocational Rehabilitation (DVR) A statewide employment resource for businesses and people with disabilities. Their mission is to enable individuals with disabilities to obtain and keep employment. Multiagency Network for students with Severe Emotional Disturbance  (SEDNET) A regional, interagency networking project, which has been established to improve the range of quality of programs for students with emotional handicaps and severe emotional disturbance in Miami-Dade and Monroe counties.
Easter Seals Miami-Dade Provides services to children and adults with or without disabilities and other special needs and their families.  Parent To Parent of Miami, Inc.  A community wide, tax exempt, non-profit organization that provides information, educational training, support, and emergency assistance, to families who have children and adults with disabilities and/or special needs.
Epilepsy Foundation of South Florida Advocates for the rights and needs of people with epilepsy and seizure disorders, including ensuring that the community creates and provides necessary support services. Social Security Administration  (SSI)The Social Security and Supplemental Security Income disability programs are the largest of several Federal programs that provide assistance to people with disabilities.
Family Care  Council of Florida (FCCF) The missions of the FCCF is to advocate, educate, and empower individuals with developmental disabilities and their families, partnering with the Agency for Persons with Disabilities (APD) to bring quality services to individuals for dignity and choice. South Florida Workforce  Provides high quality, customized employment and training solutions to workers and employers in support of Economic Development in Miami-Dade and Monroe counties.
Family Network on Disabilities of Florida (FND) Provides parent training, information and support to families of children and adults with disabilities. Spinal Cord Living-Assistance Development, Inc. (SCLAD) Facilitates independent living, economic self-sufficiency and full community integration for people with disabilities by providing and coordinating support services; and, by generating employment and vocational opportunities and affordable barrier free housing.
Florida Department of Health- Children's Medical Services (CMS)/Youth Transitions  The websites that are listed here contain information that the Children's Medical Services Network believes are useful for teens and young adults with special health care needs, as well as everyone who is working with them, as they transition to all aspects of adult life, including health, education, employment, insurance, and independence. Sites were chosen that provide Florida and/or national information.  The CMS Network hopes you will find them to be useful. The Transition Center  The Transition Center is located in Norman Hall at the University of Florida. Their goal is to be a resource for all those working in the area of transition across the state of Florida. They strive toward achieving this goal through workshops, research, dissemination of products, involvement in the state's Transition Task Force, and Project CONNECT.

 

 

 

Florida Diagnostic & Learning Resources System-South  (FDLRS-S) A special education support system for educators and other professionals who work with exceptional children.  Provides diagnostic and instructional support services to district exceptional student education programs and families of students with varying exceptionalities. United Cerebral Palsy (UCP) of South Florida  UCP programs and services are designed to meet the special needs of each individual and family served, with a common goal: to enable people with disabilities to reach their greatest potential, and achieve rewarding and productive lives as fully participating members of their communities.
