In an effort to assist parents, students, and teachers in their role as informed participants during Transition IEP meetings, the Division of Special Education-Transition Task Force has developed a series of information sheets to be shared with all stakeholders.  This will help to ensure that meaningful information on the transition process and transition-related activities are shared and will assist in facilitating better outcomes for our students with disabilities.
