Office of Special Education, Alternative Outreach, and Psychological Services (OSEAOPS) has completed a review of all exceptional student education  (ESE) forms.  Some of the forms will be accessed solely from the Miami-Dade County Public Schools website, as well as this web page.  This change in policy is due to the continuous need to update forms in order to meet state and federal requirements.  The new practice of solely accessing forms from either of the two mentioned websites will ensure that the most current and up-to- date form is being utilized districtwide.

You may access Forms Management and select "Forms Search" in order to search for any forms.  Note, you will be required to enter the FM# of any form you are requesting. Form #'s and form titles are listed below.

You may also refer to the table below and click on the Form # or Title in order to obtain an immediate copy.  The forms on this web page are listed in numerical order and may be viewed online using the appropriate Adobe Reader plug-in for your browser. The files can also be downloaded to your computer where they can be viewed or printed.   If you need Adobe Reader, go to the site and download the one for your computer platform and browser.

Please note, ESH after the title of any form signifies the form is available in English, Spanish and Haitian-Creole.  In order to access the Spanish and Creole versions of the form click on the appropriate bookmark on the left side of the form after it opens in Adobe Acrobat.
